It has been OK for thousands of years.
Look at it this way, two cousins have, 25% + 25% (i.e. the mother of the child gives 50% of chromosomes, and she has 25% from her father, 25% from her mother) of chromosomes shared from the original family, and 25% + 25% from their differing mothers/fathers, which makes 50% from the original family and 50% other. When they have children it will be 50% of chromosomes/gene information from the original family, and 50% from the other family - just like a standard random male meets random female scenario. With siblings, we have 25% + 25% from the original family, and again, 25% + 25% from the orginal family, meaning there are no new "genes" per se in the child, meaning that is inbreeding. In other words, marriage between cousins is the threshold from which incest ends/begins.
Just because it is permissible, however, doesn't mean you have to, it just means you keep your nose out or do not pass judgement in cases where it does happen. If I remember correctly, the royal families are notorious for this practice.

According to 4-23 cousin marriage is not incest, however the word , immigrated with you" in 33-50 gives an impression that , caution must be taken , A few generation's inbreeding can cause trouble and hence may be avoided. i mean do it only if it is necessary, don't make it a habit.
And this was probably not being practised at that time, that is why this was specifically spelled out on this occasion, when believing women were in short supply.

Cousins are not that bad. I would not say they are a last resort. I'd readily take a cousin as a first resort, but different people feel differently when it comes to who they allow themselves to become romatically involved with or attracted to.
