Or Is Al Masjid Al Haram not a physical masjid at all but something that can happen anywhere and everywhere ?

When that is a common law throughout the land.
It is "no fighting" full stop.

This is part of the Quranic command regarding the permissibility to fight. It always should be defensive war. The location you mentioned has some restriction in terms of fighting and hunting so the Quran gave emphasis on that since its a place of worship and pilgrimage. We are always allowed to defend ourselves.

Yes as far as my understanding of Quran. However the Masjid Al Haram has some restrictions the Quran says so when the believers were engaged in fighting with pagans the Quran tried to tell the believers to avoid battles in and around Masjid Al Haram area unless forced.
But yes even here the Quran said that if the pagans do not respect the sanctity of a place of worship and fight you there than fight them back.

evil intent , evil actions , evil goings on , evil extreme - (al-masjid al haram ) always require us to stand our ground . The biggest example was what was happening to the jews and Moses was taken from that evil place , from al-masjid al haram to al masjid alaqsa (Extreme actions of Qisaas), to a state of demanding justice for the jewish people.
Evil doers are to be prevented from causing havoc (prevented from al masjid al haram)
The Americans think President Asad used chemical weapons on his own people , Now the Americans are using their means to stop Asad - deterring him from Masjid Al Haram.
No doubt if they can't deter him from the masjid al haram them the american will also come to sought him out.

I wasn't talking about any roots , but giving a vague picture of an example.
Moses's story is a good example Almasjid Alharam to Almasjid Alaqsa - going from haram goings on to stand for the right cause is blessed indeed.
You are supposed to deter/prevent people committing evil deeds and even fight them if they then attack you.
It happens all over the world.
